A vision plan is a popular employee benefit. And UnitedHealthcare offers one of the broadest eyewear and eye care programs in the country.


Benefits include:

  • Large network of eye doctors and optical retailers

  • Access to discounts on laser surgery

  • Comprehensive eye exams (after copayment)

  • Most lenses, frames, and contacts (after copayment)

Vision benefits through Ameritas that are straightforward and easy to understand, so you know exactly what is covered.

Three plan choices are available

  1. Reimbursement. There is no network. You have the freedom to select any vision provider you choose. Then simply pay your provider and submit a claim. Ameritas will reimburse you up to your set annual dollar amount of $200. There are no frequency limitations. You have a flat amount of $200 to use for eligible exams, lenses and frames collectively.

  2. EyeMed network. Choose from a network made up of chain vision providers.

  3. Vision Service Plan (VSP). Largest network of independent optometrists and ophthalmologists.

Employers may pick more than one plan offering for employees. Plans may be employer paid, employee paid or a combination of employer/employee.
